The Case Manager is a key member of the interdisciplinary treatment team, responsible for coordinating and advocating for comprehensive services that address the biopsychosocial needs of clients in outpatient substance use and co-occurring mental health treatment. Utilizing a trauma-informed, person-centered approach, the Case Manager supports client recovery through care coordination, resource navigation, and ongoing support to address barriers to treatment engagement and retention.
