Reporting to their respective program supervisor, the Case Manager - MH serves as a case manager for assigned clients and performs related duties toward the objectives and goals of the division.

Hours:  Monday-Friday 8-4:30 with possibilities of comp time and some flexibility may be required
Office is located at 320 S. Walnut St. in Appleton, WI

As a Community Mental Health Case Manager, you'll make a meaningful difference by helping individuals navigate services and achieve their goals. Success in this role requires excellent communication, organization, and relationship-building skills to effectively support individuals with diverse and complex needs.

In your first year, you'll build a strong understanding of community resources, gain confidence in care coordination and documentation, and work collaboratively with an interdisciplinary team to provide person-centered support.
