The Bilingual Case Manager is responsible for initiating and maintaining regular (at least weekly) short-term (up to 30 days) contact/support with families. The Case Manager will provide an assessment of the family for service needs and linkage to available local community resources to prevent or remedy or assist in the solution of problems that may result in neglect or abuse to a child. This role will conduct family-centered and strength-based interventions directed at establishing a trusting relationship. Services are geared towards family reunification, maintaining, strengthening, and safeguarding families to prevent substitute care placement, the need for further involvement in the child welfare system, and ensure children''s safety and well-being.
Duties include, but are not limited to:
Conduct initial family assessments.
Ensure the safety and well-being of involved children''s services are being delivered.
Support families in resolving challenges that may increase risk to children
Conduct weekly home visits to support families and assess factors impacting child safety and well-being.
Identify, research, and regularly update community resource information to ensure referrals are relevant, accessible, and aligned with family needs.
Direct intervention to help reduce risk and teach strategies for avoiding risk in the future.
Verify families'' linkage to community resources to include documentation that the family has received and connected to the resources.
Manage the client waitlist by triaging individuals to active caseloads based on urgency and need.
Deliver short-term services to support parents in maintaining safe, organized, and clean home environments.
Connect clients to appropriate medical services based on identified health, safety, and risk needs.
Provide advocacy services to families with public systems such as DHS/TANF, public schools, and others identified through family assessment.
Maintains records and statistics required by the agency, state, and other funding sources and makes suggestions for revision or review.
The ideal candidate will have the following education and experience:
Bachelor''s in early childhood, social services, or related field.
Must write and speak fluent English/ Spanish.
The ability to speak the primary languages of the clients.
Maturity and experience in successfully working with children (0-17 years of age) and their families.
Understanding of child development and the dynamics of parent-child relationships.
Ability to approach families from a strength-based model and a family-centered service model.
Available to families after-hours on an emergency basis.
Ability to establish and maintain personal/programmatic boundaries while providing supportive services.
Experience in working with culturally diverse communities and families with the ability to be culturally sensitive and appropriate.
Reliable transportation required, including the ability to travel locally to conduct home visits and connect families to services.
Salary: $50,000/year
