As a Case Manager of Admissions / Discharges and Infection Prevention Specialist your voice to influence patient care is valued and empowered at every turn –whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.

Job Summary and Qualifications

· You will assess the process of collecting in-depth clinical information about a patient and identify individual needs in order to place the patient in this facility.

· You will plan the process of collaborating with interdisciplinary healthcare providers to determine specific objectives, goals and actions designed to meet the patient’s needs.

· You will implement the process of documenting specific case management activities/interventions that will lead to accomplishing the goals set forth in the patient’s care of plan.

· You will coordinate the process of organizing, securing, integrating, modifying and documenting the resources necessary to accomplish the goals of the plan of care and discharge.

· You will monitor the ongoing process of gathering sufficient information from all relevant sources and it’s documentation regarding the case management plan and it’s activities/ services to enable the case manager to determine the plan’s effectiveness.

· You will evaluate the process, repeated at appropriate intervals, of determining and documenting the care management plan effectiveness in reaching desired outcomes and goals.

    · You will measure the intervention to determine the outcomes of care management involvement.

    What qualifications you will need:
    • Required Experience: 1 year of recent clinical nursing experience in relevant practice area or in case management
    • Preferred Experience: 2 years of recent clinical nursing experience in relevant. 1 year management or Case Management experience
    • Required Education: Associate’s Degree in Nursing
    • Preferred Education: Bachelor’s Degree in Nursing
    • Required License: Currently licensed as a Registered Nurse in the state(s) of NC or compact state defined by practice according to law and regulation.
    • Required Certification: BCLS, Infection prevention or within 2 years of hire
