Reporting to the Director and Chief Mental Health Officer of Counseling and Consultation Services (CCS), the case manager is a key member who will support the mission and vision of the department in supporting students wellness needs.

Position Summary

The Case Manager will play a role in providing case management and in connecting students to campus and community resources to help support their psychological, social, economic, and emotional needs. The CCS case manager will provide telehealth or face-to-face care for students in need of longer-term or more specialized mental health treatment; coaching and skills-based training for students navigating insurance and mental health care options, campus and community referral follow-up, collaboration and outreach with campus partners who provide case management for students, and active collaboration with community mental health providers and agencies, including those who offer free and low cost treatment.

Essential Duties and Responsibilities

Consultation & Student Support 40%

  • Provide need/intake assessments, crisis support and consultation to students
  • Provide follow-up for students of concern as needed, including but not limited to the after-hours emergency service and AI responses
  • Serve as the primary point of contact for student requests, providing support for their basic needs such as mental and physical health, food, transportation, housing, and childcare
  • Create action plans with individual students and manage ongoing cases
  • Provide coaching and skills training to students on self-advocacy, navigating health insurance, and connecting with mental health and other wellness services

Community Support & Outreach 35%

  • Build and maintain positive collaborative relationships with college faculty, staff and outside agencies to provide care coordination and support through advocacy and referrals and maintain updated knowledge of resources and referral processes
  • Provide outreach presentations to the campus community
  • Develop and maintain comprehensive knowledge of community resources and referral databases
  • Provide support to CCS department related to crisis intervention and aftercare
  • Initiate and maintain ongoing liaison relationships with community health providers, services, hospitals, and other resources

Coordination & Case Management 25%

  • Assist in coordination of patient discharge from inpatient hospitalization, coordinating with other campus-based case managers in the Dean of Students office and Student Health & Wellness
  • Monitor and coordinate direct referrals between the Student Health Center and CCS
  • Document case management services, maintain data, and provide timely reports

Minimum Required Qualifications (SPS)

  • Masters degree in social work or related field
  • LSW, LPC, LCSW or LCPC
  • Demonstrated experience working with diverse populations.
  • Experience with identify and making referrals for appropriate services.

  • Strong interpersonal, communications, and technical skills required.

Preferred Qualifications (SPS)

  • Two (2) to Five (5) years of experience in working as a case manager in a community agency, educational institution, or related professional field.
  • Experience building relationships with external/community partners
  • Bilingual (English/Spanish)
