Position Summary: The Case Manager works as part of an interdisciplinary team to support individuals with serious mental illness/general mental health in improving their daily functioning, independence, and overall quality of life. This role focuses on active engagement and delivery of meaningful services that help individuals build skills, access resources, and maintain stability in the community. Services are purposeful, goal-directed, and aligned with each individual's treatment plan, with a clear focus on improving functioning and health plan goals.
