CCHS is dedicated to expanding access and creating a system wide treatment model utilizing proven systems of care including, but not limited to, Medication Assisted Therapy, Behavioral Health Intervention, Contingency Management, and ancillary social support structures to improve the mitigation of Social Determinants of Health that perpetuate cycles of substance use.A CARP Behavioral Health Provider, will act as part of a comprehensive treatment team utilizing patient -centered decision making. Their duties will include, but not be limited to, the completion of clinical care, for patients identified as being on their panel/caseload in addition to any other patient of CARP that needs services. CARP Behavior Health Providers may also be consulted in the development of clinical policies ensuring evidence-based treatment.Assist in identifying evidence based clinical guidelines related to addiction treatment for the teams use.Facilitate the development and implementation of Group Therapy sessions in collaboration with other CARP BHPs.Assist the CARP team to ensure program maintenance of program guidelines, internal and external.Participate in team meetings to identify daily needs, treatment planning, and facilitation of programming. Provide daily consultation to CARP team to address changes in patient treatment plans. Including, facilitating communication between CARP staff to ensure continuity of care.Participate in weekly staff meetings to address staffing concerns, and relevant treatment plans are communicated to the team as needed.Assist in expanding CARP services to other CCHS Health Centers as assigned by CARP Clinical Director.Ensure and perform appropriate and accurate diagnosis, treatment planning, and intervention of CARP patients.Remain available to provide consultation services to patients in context of same-dayreferrals as well as cross-site consultation when necessary.Provide brief, limited, and problem-focused outpatient therapy to selected patients. Collaborate with other members of the medical staff and health care teams in the provision of health care services.Coordinate appropriate referrals for patients needing more intensive or specialty services, and offer short-term support to patients until otherwise established.Collaborate with staff colleagues to review and develop clinical guidelines andprocesses and provide psycho-education pertaining to behavioral health principles.Keep and maintain appropriate and timely records relating to professional services rendered and under this agreement, which shall belong to the Health Center. Respond to patient cases and other electronic health record communications in a thorough and timely manner.Precept and supervise health professions students and psychology students/trainees.Participate in quality of care reviews and practice-based research.Participate in clinical huddles, BH team meetings, clinic leadership team meetings/activities, and other clinic-based and system-wide meetingsas requested.Collaborate with psychiatry and generalist BHPs to facilitate integration and functioning of services.Other tasks as assigned by CARP Clinical Director.Job RequirementsLicense or license eligible Social Worker, Psychologist or LPC.Experience in the treatment of Substance Use Disorders.Will be able to sit or stand for extended periods of time.Will maintain personal transportation and be able to travel regionally, and further by arrangement.
