Position Overview
Bloom Health Centers is seeking a dedicated and professional Care Team Coordinator (CTC) to join our administrative team at our Winchester, VA. This role is essential in ensuring efficient front office operations and supporting our mission of delivering compassionate and accessible psychiatric care. Bloom Health Centers also provides statewide telehealth services, and this position plays a key role in maintaining seamless coordination and communication between patients, providers, and the broader care team.
The Care Team Coordinator (CTC) is responsible for supporting front office operations and ensuring a seamless and positive patient experience at Bloom Health Centers. This includes managing incoming telephone calls and email correspondence, scheduling and confirming patient appointments, facilitating communication between patients and clinical staff, and ensuring the accurate maintenance of patient records and charts.
Key Responsibilities
Patient Experience & Support
- Provide friendly, respectful, compassionate, and accurate support to patients in person, by phone, and via email
- Serve as a primary point of contact for patients seeking behavioral health services
- Ensure patients feel welcomed, respected, and informed throughout their interactions with the office
- Address patient concerns or grievances with professionalism, empathy, and a solution-focused approach
- Escalate complex patient issues to the Office Manager and/or Regional Manager when appropriate
- Other job duties as assigned
Scheduling & Patient Coordination
- Schedule and confirm patient appointments
- Send and track patient intake paperwork and required forms
- Coordinate communication between patients, providers, and clinical staff
- Support patient follow-up communications and appointment reminders
Administrative & Documentation Support
- Maintain and update patient charts in the electronic medical record (EMR) system
- Document patient communications and interactions accurately and in a timely manner
- Ensure strict compliance with HIPAA and patient privacy standards
- Assist with prescription prior authorizations and medication refill coordination as directed by providers
Office Operations Support
- Answer and triage incoming phone calls, email, and fax communications
- Support efficient front office workflow and operations
- Assist with maintaining organized patient records and documentation
- Participate in weekly staff meetings and operational discussions
- Contribute to initiatives that support the growth and improvement of Bloom Health Centers
Team Collaboration
- Collaborate with providers, therapists, and administrative staff to support continuity of care
- Support a positive and collaborative team environment
- Demonstrate sensitivity and responsiveness to the diverse cultural, religious, and gender identities of patients and staff
Qualifications
- High school diploma or GED required
- Minimum of 1 year of customer service, receptionist, call center, or healthcare administrative experience preferred
- Experience working with electronic medical records (EHR) systems strongly preferred
- Ability to type a minimum of 35 words per minute
- Strong organizational skills and attention to detail
- Excellent verbal and written communication skills
- Ability to multitask and remain organized in a fast-paced healthcare environment
- Basic computer proficiency including Microsoft Word, Excel, Outlook, and Teams
Work Requirements
- This is a full-time, on-site position
- Must be available Monday through Friday for an 8-hour shift scheduled between clinic operating hours of 8:00AM and 7:00PM
- Occasional weekend or evening coverage may be needed
- Occasional travel between nearby Bloom Health Centers locations may be required to support operational or clinical coverage needs. Travel beyond the employee’s assigned primary work location will be reimbursed in accordance with company mileage reimbursement policies
- Reliable attendance is required as this role is classified as essential to office operations
