  • Have you provided training or given presentations to help increase awareness of student support resources?

Position Summary The CARE Case Manager 3 supports the Student Care and Outreach Team ("Care Team") in managing cases involving students of concern, with an emphasis on promoting student well-being and success. This role includes initiating cases, coordinating follow-up actions, documenting team decisions, and monitoring student progress through established action plans. The position is responsible for managing daily Care Team access points and providing status updates on active cases. Will also serve as a liaison to faculty, staff, parents, and other stakeholders seeking consultation or assistance from the Care Team. Additionally, this role may support Temporary Conditions and Withdrawal Services by coordinating accommodations and assisting students in navigating short-term or hardship-related academic adjustments. The position contributes to campus-wide awareness of Care Team services through training, presentations, publications, and data reporting.

Key Responsibilities

  • Manage a caseload of students using a system-based approach to support success and well-being.
  • Triage Care Team cases and gather information to make recommendations.
  • Meet with students to provide support, build resilience, and connect them to resources.
  • Open and manage cases; coordinate interventions; support care plans; track progress in Maxient.
  • Close cases, ensuring all plans and follow-up actions are completed.
  • Coordinate projects related to grants, basic needs, and best practices.
  • Provide outreach and training to the campus community to increase awareness and support students.
  • Develop solutions and accommodations for students impacted by injuries, surgery, or short-term conditions affecting daily functioning.
  • Support students navigating leaves of absence or retroactive withdrawals due to hardship.
  • Create trainings, publications, and data reports to educate the university community on Care Team referral process and supporting students in distress.
  • Track trends using case management data to identify gaps and growth areas.
  • Develop and administer surveys to identify process improvement measures.

Minimum Qualifications (Classified and Unclassified positions)

Preferred Qualifications

  • Experience with Student Care teams, Title IX cases and trauma-informed interviewing practice, maxient electronic database and case management.
  • The ability to balance multiple competing priorities.
  • Experience with Student Conduct and Honor Code cases.
  • Pending graduates will be considered if they will be able to obtain their degree by the date of employment.

Knowledge/Skills/Abilities

  • Able to organize data to identify problems and develop and implement realistic solutions. Communicate effectively orally and in writing.
