Care Manager RN | Full Time

Rotating Weekend and Holiday Schedule

As a Care Manager RN, you'll be a key driver of clinical coordination and patient advocacy, guiding individuals through their acute care journey and beyond. Working within an interdisciplinary team, you'll assess, plan, and implement care strategies that promote recovery, reduce readmissions, and ensure continuity across the healthcare spectrum.

Your expertise in evidence-based practices, regulatory standards, and community resources will help shape care plans that are both clinically sound and deeply personalized. This role is ideal for a nurse who thrives in a collaborative environment and is passionate about improving outcomes through strategic care management.

Key Responsibilities

  • Collaborate with the care management team and healthcare providers to coordinate patient care during acute episodes

  • Assess patients for care coordination needs and facilitate the physician's plan of care

  • Develop and implement discharge plans that support recovery and continuity of care

  • Monitor and evaluate services and options to meet individual health needs

  • Apply evidence-based practices and utilization review standards to guide care progression

  • Coordinate with community resources and external providers to support post-discharge needs

  • Communicate effectively across disciplines to ensure optimal clinical, financial, and patient satisfaction outcomes

  • Navigate electronic medical records and contribute to documentation that supports regulatory and revenue cycle requirements

Minimum Qualifications

Education

  • Associate Degree in Nursing from an accredited institution required

  • Bachelor's Degree in Nursing preferred

Experience

  • Minimum of 2 years of medical acute care experience required

  • Relevant experience in acute care, skilled nursing facilities, physician offices, or community health settings strongly preferred

Licensure/Certification

  • Current RN license in the State of Maryland, or a Compact RN license with Multi-State Privilege designation

  • Must obtain care/case management certification within 5 years from an accrediting body

  • RN licenses designated as "Single State" from compact states do not meet requirements; Maryland licensure is required

Knowledge, Skills, and Abilities

  • Strong understanding of utilization review criteria and evidence-based practices

  • In-depth knowledge of disease processes and regulatory requirements

  • Familiarity with insurance regulations and agencies such as CARF, JCAHO, and the State of Maryland

  • Proficiency in electronic medical records and computer systems

  • Ability to multitask and navigate complex care environments

  • Knowledge of community resources and the local medical staff network is highly beneficial

  • Commitment to patient-centered care and continuous quality improvement
