Summary
The Care Manager, Retention is responsible for developing and maintaining relationships with clients and caregivers to ensure retention, compliance, and quality-of-care. This role will manage long-term client relationships and satisfaction, which includes conducting home visits, monitoring EVV compliance and managing EVV barriers, Plan of Care annual updates, Caregiver evaluations (90 day and annual), incident reporting, post-hospitalization follow-up/Care Gaps, and other duties as assigned. Strong communication, problem-solving skills and initiative to be present in the community are necessary for this role to be successful. The Care Manager, Retention reports to the Associate Director of their Office.

May require sitting, standing, or walking on a level surface for periods throughout the day. May require lifting up to 20 pounds. Regional travel up to 50% required to other JEVS locations, home visits and other related functions.
Benefits
The base pay for this position ranges from $45,000 to $52,000 and will vary based on factors such as skills and experience. Base pay is one part of the Total Compensation that JEVS Care at Home provides to recognize employees for their work.
