    Join Our Team: 

    Esse Health is searching for compassionate Nurse Practitioner Care Managers to join our team!  

    Opportunity: 

    • PRN schedule.  
    • Let us know what days and hours you would like to work! 
    • Monday - Friday during normal business hours. 
    • No nights, weekends or holidays.
    • Not benefit eligible. 
    • 2 or more years experience as an Adult, Family or Gerontology Nurse Practitioner, is preferred.
    • Position will be housed out of our HUB/Central office located at 555 Maryville University Drive. 

    Day-to-Day:

    • Focus on assessment, diagnosis, treatment, care coordination, and chronic disease management for patients in their homes or via telehealth platforms. 
    • Provide service through core competencies of direct care, consultation, expert guidance, leadership, ethical decision making and collaboration.
    • Use your advanced clinical skills and training to assess, diagnose, manage, and treat patients.
    • Collaborate and participate in care coordination activities that meet the patients' needs.
    • Reduce barriers when managing discharge planning, directing transitions of care, and assisting patients throughout their health care journey.
    • Work collaboratively with physicians, care management teams, home health agencies, and other healthcare professionals to ensure high-quality, timely, and accessible care. 

    Qualifications:

    • Master’s Degree of Science in Nursing 
    • Current Nurse Practitioner licensure in Missouri (or eligibility)
    • Certified Nurse Practitioner (or eligibility)
    • Strong clinical, communication, and interpersonal skills
    • Commitment to patient-centered, team-based care
    • Ability to work independently
