Shift Details : Mon – Fri, days 35hrs/week
Job Description:
- Perform LTSS and SDOH assessments to evaluate member needs
- Coordinate care services including appointments, transportation, medications, and DME
- Document all interactions accurately in DCMS and related systems
- Coordinate and send plans of care (POCs) to acute care facilities for admitted members
- Notify primary care providers (PCPs) of patient admissions and discharges
- Perform timely and accurate data entry in the Disease Care Management System (DCMS).
- Conduct monthly member outreach calls in Spanish, ensuring HIPAA-compliant verification
- Bilingual in Spanish required.
