In this role, you aren't just managing schedules; you are the architect of a premium care experience. You will ensure operational excellence, mentor our caregivers, and provide families with the peace of mind they deserve.
As a Care Manager, your mission is split between clinical oversight, relationship management, and operational precision.
Client & Family Advocacy
Seamless Transitions: Lead the "Start of Care" process, ensuring staffing and communication are flawless from day one.
Relationship Management: Act as the primary liaison for families, conducting regular check-ins and coordinating care conferences (including hospice care teams).
Care Planning: Continually review and update care plans and logs (including LTC) to ensure they reflect the evolving goals of our clients.
Medication Oversight: Manage Mediset refills and ensure timely completion.
Caregiver Leadership
Mentorship & Quality: Conduct supervisory visits and annual performance reviews to ensure every caregiver exceeds Endeavor's high standards.
Training: Lead skills assessments for new hires in our lab and provide supplemental field training.
Accountability: Manage introductions between new clients and caregivers; issue performance warnings or "CON" (Concerns) when necessary to maintain excellence.
Operational & Community Excellence
Concierge Management: Oversee all concierge-related care, building deep relationships with key contacts at aligned senior living communities.
Growth: Handle inquiries/intakes and participate in monthly marketing events to grow the Endeavor footprint.
Responsiveness: Participate in the on-call rotation and handle administrative essentials like billing distribution and phone triage.
The Connector: You have an innate ability to build trust with families and command respect from care teams.
The Problem Solver: You can navigate the complexities of home care operations and "think on your feet" during on-call situations.
The Professional: You are organized, detail-oriented (especially with care logs and billing), and possess a "concierge-level" service mindset.
The Expert: Proven experience in home care management, senior living, or a high-level clinical supervisory role.
