This is a telephonic position serving members who are on CST time. Availability from 8am-4:30pm, Monday-Friday CST is required.
15+ welcome calls & health risk assessments per week, emergency department follow up/care transition coordination, care plan development, coordination with team.

Job Description:
15+ welcome calls and health risk assessments per week, emergency department follow up/care transition, care plan development, coordination with other team members. Territory: Telephonic role with contact of members within current 44 county service area
Charting/Note software: MIDAS.
Additional view only access for various health system EMRs (UW, SSM) and WISHIN. Caseload: No assigned caseload, only assigned for assessment completion with handoff to assigned team member for continued case management as needed.
Assessment requirement:
1 telephonic assessment/year/member.
Contact attempts to complete/schedule will vary, with a minimum of 3 outreach phone call attempts per member
Case population: Medicare + Medicaid recipients
Must have reliable internet.
Must Have Skills:
Experience requirement: strong documentation, communication and member engagement skills;
ability to identify and appropriately escalate any member concern identified during welcome calls/HRA completion process.
Knowledge of Medicare benefits/supplemental benefits, ability to navigate online provider directory and offer members in-network provider options; navigate supplemental benefit vendors and provide members with contact information.
