We are a rapidly growing home care organization in Chattanooga, seeking a compassionate, hands-on Care Manager to support clients and caregivers while helping to drive operational excellence.
What You’ll Do
- Conduct client assessments and create individualized care plans.
- Perform in-home visits, reassessments, and provide ongoing client support.
- Supervise, train and coach caregivers to maintain quality and service standards.
- Introduce caregivers to clients, review expectations, and ensure consistent care delivery.
- Maintain accurate documentation and coordinate with community providers.
- Conduct initial intake conversations with the prospective client or family member/ guardian to assess needs and determine service fit.
What We’re Looking For
- 2+ years of experience in home care, home health, or a related field.
- Strong communication, organization, and follow-through skills.
- Ability to supervise caregivers and advocate for clients.
- Reliable transportation, valid driver’s license, and insurance.
- Professionalism, compassion, and dedication to serving others.
