Care Manager

We are seeking a dedicated Care Manager to join our team and provide exceptional care coordination and management services.  

Responsibilities: 

• Collaborate with clients, families, and healthcare professionals to develop customized care plans. 
• Conduct assessments to determine individual client needs and preferences.  
• Coordinate and schedule caregiving services based on client requirements. 
• Supervise and train caregivers to ensure quality care delivery. 
• Regularly perform client visits to monitor care quality and address any concerns. 
• Maintain accurate and up-to-date client records and documentation. 
• Act as a liaison between clients, families, and healthcare providers to facilitate effective communication and optimal care management. 
• Stay updated with industry trends and best practices in care management. 

Compensation:

• Annual Salary of $40,000 to $45,000

Requirements: 

• Strong interpersonal and communication skills 
• Experience in care management or similar role 
• Empathy and understanding of the needs of seniors and their families 
• Ability to effectively coordinate and manage multiple tasks 
• Proficient in using care management software and technology 
• Valid driver's license and reliable transportation
