At Paving the Way MSI, you will do more than coordinate services—you will help children, adolescents, families, and healthcare providers navigate behavioral health systems and access the support they need to thrive.
As a Care Coordinator within our DC Mental Health Access in Pediatrics (DC MAP) program, you will serve as a critical link between primary care providers, behavioral health professionals, community resources, and families. Your work will help ensure children and youth receive timely access to behavioral health services while supporting improved outcomes and stronger care coordination.
Many of the families we serve face challenges related to behavioral health concerns, developmental needs, social determinants of health, and barriers to accessing care. Through your work, you will help families navigate resources, overcome obstacles, and connect with services that support their well-being.

What You'll Do
As a Care Coordinator, you will help connect children, families, healthcare providers, and behavioral health resources to ensure timely and coordinated care.
You will:
• Coordinate behavioral health referrals and support families in accessing appropriate services
• Collaborate with primary care providers, behavioral health professionals, schools, and community organizations
• Educate families about behavioral health resources and available support services
• Help identify and address barriers that may impact access to care
• Develop and maintain care plans that support positive outcomes for children and families
• Conduct follow-up outreach to monitor progress and support successful service engagement
• Coordinate care transitions and discharge planning activities
• Support high-risk referrals and complex care coordination needs
• Maintain accurate documentation and service records
• Participate in multidisciplinary meetings and quality improvement initiatives
Required Qualifications
• Bachelor's Degree in Social Work, Psychology, Public Health, Human Services, or a related field
• Strong communication, relationship-building, and organizational skills
• Ability to manage multiple priorities and work independently
• Commitment to family-centered, culturally responsive, and trauma-informed practices
Preferred Qualifications
• 2–3 years of care coordination, case management, healthcare navigation, or behavioral health experience
• Experience working with children, adolescents, and families
• Familiarity with community resources and behavioral health systems
