POSITION SUMMARY: This position is responsible for assisting staff in providing the necessary support and information needed to implement each clients treatment plan. The incumbent also assists clients with daily activities, medication monitoring, management of daily tasks and providing transportation when needed. Crisis management and support is also an important function of this position.
MINIMUM QUALIFICATIONS: Scale 1 - High school diploma or equivalent, demonstrated written and verbal skills, knowledge of practical self-care skills, ability to deal effectively with persons with mental illness, current PA drivers license and access to a vehicle during work hours. Scale 2 - Bachelors Degree in a related field; Or Associates degree in a related field with two years full time (or equivalent) related experience; Or High school diploma (or equivalent) with eighteen months full time experience as a Rehabilitation Technician One (Scale 1) within the agency; Or A high school diploma (or equivalent) and three years of related full time (or equivalent) experience from outside the agency. Scale 3 - Bachelors degree in a related field and at least twelve months experience as a full time Rehabilitation Technician One and/or Rehabilitation Technician Two within the agency; Or An Associated degree with at least three years full time direct care experience within the agency; Or A high school diploma with at least 48 months experience as a Rehabilitation Technician One and/or Rehabilitation Technician Two within the agency; or A Bachelors degree in a related field with eighteen months of full time (or equivalent) experience from outside the agency; Or Associates degree in a related field with at least thirty months of related experience from outside the agency. Valid drivers license with access to a vehicle during scheduled work hours.

POSITION SUMMARY: The Mental Health Professional (MHP) functions as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to clients enrolled in the ACT program. Responsibilities include, but are not limited to, the following: case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each clients individual treatment plan and maintain professional relationships with other providers, both internal and external to the program.
MINIMUM QUALIFICATIONS: Masters level degree & licensure in related field. Exceptional communication skills, both written & verbal, organizational skills & the ability to effectively work with a team of other mental health professionals. Strong clinical skills with working knowledge of the mental health system, psychiatric rehabilitation principles and recovery-oriented practices. Ability to effectively work with persons with mental illness, developmental disabilities, substance abuse disorders and forensic issues and encourage their participation with comprehensive treatment plans. Act 33/34/31 Clearance and FBI Criminal Background Check. Valid drivers license with access to a vehicle during scheduled work hours.

POSITION SUMMARY: The Registered Nurse (RN) functions as a clinical member of the multidisciplinary team and provides recovery oriented treatment, rehabilitation, and support services to clients enrolled in the ACT program. This position is responsible for assessing the physical and mental health needs of each program participant in conjunction with the psychiatrist, which includes planning medical interventions for inclusion in the treatment plan, providing health education, making appropriate referrals to community physicians and maintaining regular contact with the provider pharmacy. The incumbent is also responsible for management of the medication monitoring service which includes maintaining appropriate medication inventory, storage, and security systems; ensuring that all MARs are accurate and up-to-date;, and maintaining up-to-date written and verbal record keeping. Communication with other program staff regarding the clients medical conditions and medication needs is also critical. The RN, along with every member of the ACT team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.
MINIMUM QUALIFICATIONS: Current licensure as Registered Nurse by State of Pennsylvania; experience working with individuals with mental illness, developmental disabilities, substance abuse disorders, and/or forensic issues preferred.
