We are home to the only trauma center in New London and Windham counties and are the only area hospital with LIFE STAR helicopter services. We deliver and coordinate a continuum of high-quality healthcare with the goal of improving the health of our communities.

JOB SUMMARY

Care Management is a collaborative practice model including patients, nurses, social workers, physicians, healthcare team members, caregivers and the community. The Care Management process encompasses communication and facilitates care along a continuum through effective resource coordination. The goals of the Care Management Team include the achievement of optimal health, access to care and appropriate utilization of resources balanced with the patient's right to self-determination. Incumbents of this position are professional Licensed Masters in Social Workers that utilize their education, experience, medical social work skills and the Social Work Process to determine and facilitate the most appropriate level of care and identification of discharge needs.

Qualifications

  • Masters in Social Work from an accredited school of Social Work
  • Minimum of 3 years of Medical Social Work experience . Acute Care experience preferred.
  • Licensed Masters in Social Work in the State of CT (LMSW)
  • Case Management certification ACM or CCM strongly preferred.
  • Bi-lingual - Spanish preferred

Knowledge, Skills and Ability Requirements

  • Delivers services that provides a better experience for the patient and stakeholders including but inclusive the family, physician, others as appropriate while maintaining patient confidentiality
  • Demonstrates compassion and sensitivity to the age and cultural specific to the patient, their needs for comfort, privacy and generalized care
  • Promotes and contributes, in a positive manner, to inter- and intra-departmental relationships to ensure all needs of the patient are met.
  • Strong Communication and Interpersonal Skills
  • Knowledge of Social Work Process
  • Ability to manage multiple priorities and projects form initiation to completion within prescribed schedules and utilization of resources.
  • Ability to effectively utilize a variety of Microsoft Office, Outlook and Epic software, Morrissey and Web based programs required.
  • Skilled in Motivational Interviewing
  • Ability and willingness to follow the mission and values of Backus Hospital on a daily basis
