Care Coordinator

Position Overview

We are seeking a highly motivated and compassionate Care Coordinator who can wear multiple hats-managing caregiver schedules while actively seeking and onboarding new clients. This is a key role for someone passionate about home care, committed to ensuring our clients receive the highest standard of support and service.

Key Responsibilities

Coordination

  • Match clients with appropriate caregivers, ensuring availability, skill alignment, and compatibility
  • Maintain and manage caregiver schedules, including adjustments for changes, call-offs, and coverage gaps
  • Ensure compliance with agency procedures and Illinois non-medical home care regulations
Sales & Client Acquisition

  • Actively seek new clients through outreach, community engagement, and partnerships
  • Conduct intake consultations and present service offerings to prospective clients and families
  • Build and maintain strong relationships with hospitals, senior living communities, clinics, and local organizations
  • Track leads, conversions, and monitor client satisfaction
Qualifications

  • High school diploma or equivalent required; associate or bachelor's degree preferred
  • Minimum 2 years of experience in caregiving, scheduling, or coordination within a home care or healthcare setting
  • Previous experience in sales, client intake, or community outreach highly preferred
  • Familiarity with Illinois non-medical home care regulations
  • Strong written and verbal communication skills
  • Tech-savvy; proficient in scheduling platforms and mobile applications
  • Reliable transportation, valid driver's license, and proof of insurance (local travel required)
