Care Coordinator Highlights
  • The pay for this position is competitive and based on experience
  • Full-time, 40 hours per week
  • 100% Remote / Work From Home
  • Must be able to work any 8-hour shift between 7:00 AM - 7:00 PM CST as assigned
  • Training schedule: 8:00 AM - 5:00 PM CST
  • Training typically lasts for the first 4-6 weeks of the assignment
  • Virtual attendance during training is mandatory
  • This position is a contract assignment

Responsibilities
  • Create and complete accurate referrals and applications in a centralized customer relationship management platform
  • Stay updated on policy and procedural changes while processing patient-related documentation
  • Resolve patient questions and concerns regarding status in the patient journey
  • Enter detailed, accurate, and clear information into proprietary systems while speaking with patients by phone
  • Place outbound calls for patient follow-up and confirmation activities
  • Provide an empathetic, professional, and patient-centered experience during each interaction
  • Help guide conversations while educating patients and maintaining service quality
  • Interact with referral sources, including healthcare providers, to process new applicants
  • Manage patient accounts from initial contact through final approval or denial
  • Follow company procedures, productivity expectations, and attendance requirements

Requirements
  • High School Diploma or GED required
  • 2-4 years of related customer-facing experience and/or training preferred
  • Previous experience in a call center or customer service environment highly preferred
  • Strong communication skills, both written and verbal
  • Strong attention to detail and ability to maintain accurate documentation
  • Ability to multitask across multiple software systems while speaking with patients and healthcare staff
  • Ability to type at least 40 WPM
  • Ability to accurately type notes while actively speaking with customers
  • Strong experience with Microsoft Office products
  • Comfortable using Microsoft Teams, Zoom, and other video conferencing tools
  • Ability to work independently, manage time effectively, and perform well in a high-volume environment
  • Professional, empathetic, and patient-focused communication style
  • Strong work ethic and commitment to daily and weekly performance goals
  • Knowledge of insurance billing methods and general pharmacy operations is a plus
  • Must strictly adhere to attendance expectations

Remote Requirements
  • Must have a dedicated, quiet, private, distraction-free workspace
  • Must maintain high-speed internet
  • Company will provide computer, technology, and equipment needed for the role
  • Candidate is responsible for providing internet service that meets the following requirements:
    • Secure broadband connection via DSL, Cable, or Fiber
    • Download speed: at least 15 Mbps
    • Upload speed: at least 5 Mbps
    • Maximum ping rate: 30 ms
    • Must be hardwired to the router
    • Must use a surge protector with network line protection
  • Dial-up, satellite, Wi-Fi, and cellular connections are not acceptable
  • Inability to meet remote work requirements may result in termination of assignment

  • Deliver palliative care and related health care services to children and families
  • Oversee care coordination and health coaching for the patients
  • Manage assigned panel of chronic care patients
  • Ensure that patients are receiving appropriate care

  • Receiving quality care from caregivers
  • Contacting patients to schedule transitional care services and home visits
  • Identify progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care
  • Identify the primary care team involved in the specialty patient care
  • Utilize assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination
  • Promote adherence to a care plan
  • Connect patient back to primary care physician and primary care coordinator team
