Palms Home Care is a growing home health agency dedicated to providing quality in‑home care to patients in the communities we serve. We're looking for a motivated Business Development Liaison to build and strengthen relationships with referral sources and help us reach more patients who need our services.ResponsibilitiesCultivate and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sourcesEducate referral partners on Palms Home Care's services, admission criteria, and the referral processTrack territory activity, referral trends, and conversion rates; report on growth opportunitiesCoordinate with intake and clinical teams to ensure smooth, timely admissionsRepresent Palms Home Care at community events, health fairs, and networking functionsIdentify new business opportunities and help develop strategies to expand market share within assigned territoryQualifications1+ years of business development, marketing, or liaison experience in home health, hospice, or a related healthcare setting (required or strongly preferred)Established relationships with local referral sources a plusStrong communication and relationship‑building skillsSelf‑motivated, goal‑oriented, and comfortable working independently in the fieldValid driver's license, reliable transportation, and willingness to travel within assigned territoryKnowledge of home health regulations, PDGM, and the referral/intake process preferredBenefitsHealth, Dental, Vision InsuranceCar AllowancePaid HolidaysPaid Time offThis position requires a background check.
