Job Description:
Location: 601 E. Rollins Street, Orlando, FL 32803
Full Time Monday- Friday 8am-4:30pm, with some weekend events
Assesses patients' and families' psychosocial risk factors through comprehensive social assessments and evaluation of support systems.
Completes assessments based on information from multiple sources, including patients, families, nursing staff, physicians, and multi-disciplinary rounds.
Advocates for patients in obtaining and negotiating resources to meet their needs. Collaborates with the interdisciplinary team to address barriers impacting or delaying the patient's treatment plan. Provides patient and family education, adjustment-to-illness counseling, grief counseling, and crisis intervention.
Offers end-of-life support to patients and their families. Provides consultation and educational programs for healthcare team members on the impact of psycho-social factors on illness. Facilitates staff emotional debriefings and family meetings. Serves as a resource for patients, physicians, and allied health professionals, providing education and consultation.
Coordinates necessary ancillary or consultative services to support outpatient clinics and community education.
Stays abreast of changing social work clinical trends and regulatory matters. Adheres to the National Association of Social Workers (NASW code of ethics. Other duties as assigned.
