Alignment Health is seeking a compassionate, organized, and motivated Bilingual Spanish Health Coach & Educator to support members throughout Maricopa County, Arizona.

This is a highly field-based position focused on conducting in-home visits with members across the community. The Health Coach & Educator builds trusted relationships with members, caregivers, and families by providing wellness coaching, health education, support, and encouragement to help improve overall health and well-being.

Success in this role requires comfort working independently in the field, managing a daily schedule of member appointments, and traveling throughout an assigned service area. What to Expect in This Role

  • Conduct approximately 6-8 in-home member visits per day
  • Spend the majority of your workday in the field meeting members in their homes
  • Build trusted relationships with members, caregivers, and families through health coaching and education
  • Work independently while managing a daily schedule of member visits
  • Daily local travel is an essential component of this role
  • Member visits are generally organized by geographic service areas or quadrants to support efficient scheduling and reduce driving time
  • Mileage reimbursement is provided from the time you leave home until you return home
  • Schedule: Monday - Friday, 8:00 AM - 5:00 PM Mountain Time

Candidates should be comfortable driving throughout their assigned service area and conducting multiple in-home visits each day.

GENERAL DUTIES/RESPONSIBILITIES

  1. Contacts patients that are eligible for Alignment's Care at Home program and provides necessary wellness coaching to reduce or eliminate high-risk behaviors

  2. Collaborates with advanced practice clinicians and/or social worker to develop an action plan

  3. Encourages adoption of habits that are conducive to a higher quality of life

  4. Responsible for delivering a high level of patient service and increasing member retention and satisfaction

  5. Assists patients with accessing community resources and social services to address social determinants impacting health and wellbeing

  6. Develops and distributes health education materials to identified patients, caregivers, and/or families

  7. Coordinates Care with other Alignment team members, including health-related vendors for optimal patient care

Minimum Requirements:

Experience:

  • Required: Minimum (1) year of experience as a wellness or health coach or educator
  • Preferred: Senior care experience.

Education:

  • Required: High School Diploma or GED. Associate's degree or two years additional experience in lieu of education.
  • Preferred: Bachelor's Degree

Specialized Skills:

  • Required:

  • Able to communicate positively, professionally and effectively with others; provide leadership, teach and collaborate with others.

  • Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise

  • Knowledge of and experience with applying the stages of change or a similar behavior change model

  • Excellent interpersonal skills with the ability to foster trust and respect among customers and coworkers.

  • Able to work independently and remotely.

  • Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies

  • Proficient computer skills including Microsoft Office Excel, Word and PowerPoint

  • Strong time management and route-planning skills to efficiently manage multiple home visits throughout the assigned territory.

  • Comfortable working independently in a field-based environment with frequent local travel.

  • Ability to navigate changing daily schedules and prioritize member visits across multiple coverage areas.

  • Effective written and verbal communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors;

  • Mathematical Skills: Able to perform mathematical calculations and calculate simple statistics correctly

  • Reasoning Skills: Able to prioritize multiple tasks; advanced problem-solving; ability to use advanced reasoning to define problems, collect data, establish facts, draw valid conclusions, and design, implement and manage appropriate resolution.

  • Problem-Solving Skills: Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.

  • Preferred: Bilingual English and Spanish

Licensure:

Required:

  • Current, valid, unrestricted driver's license

  • Reliable personal transportation for daily travel throughout the assigned service area

  • Ability to maintain automobile insurance in accordance with company requirements

  • Ability to travel locally throughout Maricopa County and transport necessary work materials

Pay Range: $44,987.00 - $67,480.00

Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
