RN Nurse/Home Visitor (Bilingual-Spanish/English) The Registered Nurse/Home Visitor is a grant-funded position in partnership with the Baltimore City Health Department. Under the direction of the Director of Maternal and Child Health and Clinical Supervisor, the Home Visitor completes the training for the National Nurse Family Partnership (NFP) Program.
The position builds and maintains a caseload of 25 families, composed of adolescents, pregnant for the first time, who enroll in the program prior to 28 weeks of pregnancy. After the birth of the baby, the mother and child will be followed by the prescribed NFP program until the child's second birthday.
Schedule/Hours: Monday - Friday: 8:00 AM - 5:00 PM
Essential Functions: • Actively recruits clients, in accordance with NFP standards, until building a complete caseload of 25 families. • Conducts home visits in accordance with NFP standards. • Documents all visits as required in ETO Database for the Health Department. • Records all patient contacts in BMS Electronic Health Record via the Enabling Services Template. • Reviews the Prenatal Risk Assessment completed at the client's first prenatal visit to determine risk factors and consults with Nurse Supervisor of BCHD regarding eligibility for enrollment. • Attends BMS OB Rounds and maintains contact with BMS OB and Peds providers to facilitate communication regarding patient needs and progress. • Attends monthly the BabyStat meetings of Baltimore City for Perinatal Home Visitors.
Education and Certification: Bachelors Degree in Nursing Active Maryland RN License Active CPR Certification
Experience and Skills: Experience in community health and/or working with underserved populations preferred. Fluency in Spanish highly required.
