Key Responsibilities
Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members
Perform telephonic and face-to-face assessments and ongoing care coordination
Develop, implement, and monitor care plans to support member wellness outcomes
Collaborate with PCPs and provider teams to coordinate services and supports
Support prior authorizations, medication reviews, condition management, and community resource linkage
Identify and escalate quality of care concerns through established channels
Use motivational interviewing & member engagement strategies to drive adherence & healthy behavior change
Maintain documentation & compliance with all guidelines

· Travel: 50% to 75% local travel for member visits (mileage reimbursed)
Pay: $34.13/hr.
Schedule: Mon-Fri, 8:00am-5:00pm. 4-6 weeks remote training (Microsoft Teams)
