Division and Program Description
Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion. This multilingual and multicultural outpatient program offers comprehensive services for Seniors with Mental Health concerns, helping them to maintain independence and dignity to age in place. Geriatric Outpatient Services West (GSW), based at 1500 Franklin St. San Francisco, offers mental health services to older adults with sever psychiatric disorders and includes a range of outpatient services, including treatment of dual diagnosis and homeless individuals. Under contract with the Department of Public Health – Behavioral Health Services (BHS), the GSW program provides city-wide services to older adults needing specialty mental health services. The agency is committed to implementing Evidence Based Practices and Behavioral Health research into its community-based mental health services.
Job Description
Under supervision of the Program Director, the clinical case manager participates in a multidisciplinary team serving older adults, aged 60 and over, with severe psychiatric disorders and co-occurring disorders, providing comprehensive and integrated treatment services, including mental health and substances abuse treatment, case management, and other service connections. It employs an inter-agency, multidisciplinary approach to provide comprehensive wellness/recovery-based services. Referrals for GSW come from community partners and self-referrals. The GSW clinician evaluates needs of clients and develops treatment plans, often along with the medication support team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education. The GSW clinical maintains accurate detailed observation and progress records for electronic billing data entry. Additional duties as needed.
Specific Responsibilities
- Work closely with interdisciplinary team of clinical case managers, psychiatric nurse practitioners, psychiatrist and community specialist to provide specialty mental health services to older adults.
- Provide crisis intervention to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
- Perform intakes and assessments for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic, community and/or in their homes.
- Provide individual or group therapy to stabilize clients and prevent de-compensation.
- Provide clinical case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical and other needed services. Coordinates with other service providers.
- Maintains clinical records of each client contact or service in accordance with mandated standards of Medi-Cal, Medicare, and San Francisco’s Community Behavioral Health Services.
- Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Commits to ongoing training by participating in Felton’s training and BHS trainings, including developing specializations in evidence -based treatments and practices.
- Commits to ongoing training in the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.including developing specializations in evidence-based treatments and practices and the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.
- Provides information regarding community resources to the general public as needed.
- Other duties and responsibilities that may be assigned from time to time.
Minimum Requirements and Qualification
- Master's degree or higher in Social Work, Counseling or Psychology, registered with CA BBS.
- Experience in case management and mental health treatment for at risk adults, including co-occurring disorders; experience with older adults preferred.
- Bilingual in Cantonese and Mandarin is REQUIRED.
- Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills.
- Ability to work cooperatively on a multi-disciplinary team, and to work with clients of diverse backgrounds.
- Experience, knowledge, and familiarity of the DSM 5, understands how to appropriately diagnose.
- Valid CA Driver’s Licensed preferred; travel to homes and other health/community organizations required.
- Strong organizational, interpersonal, and time management skills and attention to detail.
- Strong communications skills both verbal and written.
- Ability to prioritize projects and follow through.
- Ability to be a team player with a welcoming attitude.

Salary Range: $93,000-$105,000 Annually

This position is union-represented by SEIU Local 1021 and is subject to the applicable collective bargaining agreement.
