Title: Bilingual Clinical Care Coordinator Department: Nursing Status: Exempt Position Classification: Category Administrative and Clinical Reports To: Assistant Medical Director Direct Reports: None

Summary of Position Under the supervision of the Associate Clinical Officer, the bilingual clinical care coordinator helps to facilitate patient care in the brick and mortar and in the mobile clinic environment. She is an essential part of the care team from the onset of the visit until its completion. She is one of the primary point of contacts for mobile clinical support related matters.

Tasks assigned to this role are both clinical and clerical. She will be expected to work clinically as a certified medical assistant on designated days for the Associate Clinical Officer. In the event the mobile unit is short a medical mobile tech, she will be expected to work as a CMA. Also, she will be expected to work clerically on designated days as an administrative assistant and care coordinator for Spanish-speaking patients and Special populations.

She will be responsible for completing all administrative assistant tasks to support the Associate Clinical Officers office, such as scheduling and ensuring adequate mobile clinic staff coverage. In addition, she will serve as back-up administrative support to the Chief Medical Officers Executive Assistant if needed. She will serve as a Spanish interpreter if an interpreter is not available at mobile-related events.

The bilingual clinical care coordinator is responsible for capturing and documenting clinical and demographic information and accurately documenting this material in the electronic health record for the Associate Clinical Officer. The bilingual clinical care coordinator is responsible for effective communicating with patients, providers, and staff in multiple settings that involve the Associate Clinical Officer.

Minimum Qualifications

  • Interacts in a professional and patient-centered manner with patients.
  • Able to work cooperatively with internal staff to deliver safe, effective, quality care to all patients.
  • Able to plan, prioritize, and complete assigned tasks with a high level of accuracy.
  • Ability to work well under time constraints while maintaining accurate records.
  • Experience: 2 years of CMA experience preferred.
  • Additional Skills Required:
  • Knowledge of medical office protocols/procedures.
  • Knowledge of medical terminology.
  • Familiarity with basic computer operations.
  • Proficiency in Microsoft Excel, Word, and Outlook.
  • Good oral and written communication skills.
  • Travel Required: Ability to travel to all CRCHC locations and assist at all CRCHC special population clinics.
  • Additional Skills:
  • Bilingual in Spanish required.
  • Experience in preceptoring/training clinic or clerical staff.
  • Must be organized and attentive to details.
  • Ability to time manage and prioritize work.

Education

  • High school diploma or GED

Certifications/Licensure

  • Certified Medical Assistant
  • CPR and BLS required

Key Responsibilities

  1. Fulfill patient care responsibilities as assigned that may include checking schedules and organizing patient flow to ensure patient visits are started in a timely manner; greet and escort patients to clinical area; prepare for the provider-patient encounter by obtaining a comprehensive health history and vital signs; perform required screenings per established guidelines.
  2. Maintain accurate documentation in the patients electronic health record based on established documentation procedures.
  3. Fulfill clinical clerical responsibilities as assigned, including but not limited to the following: obtaining lab/X-ray reports, hospital notes, and referral information; complete forms/requisitions as needed; verify patient demographics; process and schedule STAT and urgent referrals; communicate to patients appointment date and time.
  4. Complete all administrative clerical duties to maintain administrative support to the Associate Clinical Officer. Duties include a variety of tasks but not limited to the following: data entry, filing, copying, faxing, scanning, in-house mail distribution, maintenance of meeting minutes, telephone answering, making appointments and travel plans, preparing reports, gathering and securing confidential information, the usage of various office equipment and computer software programs, and telephone and internet communication.
  5. Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.
  6. Helping individuals, families, groups, and communities develop their capacity and access to resources, including health insurance, food, housing, quality care, and health information.
  7. Assist Associate Clinical Officer, Chief Medical Officer, and Mobile Unit Provider with communicating with Spanish-speaking and/or special populations (i.e., Salvation Army, Patterson Farms, etc.) regarding medications, symptoms, referrals, appointments, and paperwork.
  8. Will serve as Executive Assistant to the Chief Medical Officer in the event the executive assistant is out of the office or a particular project requires additional assistance.
  9. Ensure culturally and linguistically appropriate services.
  10. Represent the company professionally and positively to enhance and promote the core values and mission of the organization.
  11. Setting up and taking down mobile medical clinics at outreach sites if medical mobile technicians are not available.
  12. Complete basic health assessments, outreach medical case management, referrals, arrange transportation to and from the clinic, and follow-up to special populations, community, and their families.
  13. Participate in mandatory in-services and drills, attend staff meetings, and other trainings, including appropriate age-specific educational trainings offered on a regular basis for professional development.
  14. Maintain strict adherence to infection control guidelines established by the organization.
  15. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving, and/or controlling program goals, objectives, and/or services.
  16. Complete in a timely manner the assigned clinic maintenance tasks, including but not limited to the following:

    • Setting up instruments and equipment, including a working knowledge of all equipment and the ability to train other personnel on its use.
    • Running tests and controls on equipment and the ability to teach other personnel.
    • Cleaning exam/procedure rooms and documenting completed tasks based on clinic protocols and the ability to train others in the appropriate procedure and techniques.

    • Assist the Associate Clinical Officer, Chief Medical Officer, Mobile Unit Provider in assuring supplies are ready and that inventory of supplies is maintained to meet the needs of the clinic and provide support to other clinics as needed.
    • Keeps equipment operating by following operating instructions, troubleshooting breakdowns, maintaining supplies, performing preventative maintenance, and calling for repairs.
    • Maintain safe, secure, and healthy work environment by following and enforcing established policies and procedures and the ability to professionally encourage others to do the same to support a healthy team atmosphere.
    • Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.

    • Able to perform COVID-19 rapid testing, specimen handling, and processing. Must be able to input results in electronic health record and communicate abnormal results to medical provider. Must be able to run controls on equipment and have the ability to teach other personnel in the brick and mortar and mobile setting in the event the Mobile Lead MA is not available. Cleaning exam/procedure rooms and documenting completed tasks based on clinic protocols and the ability to train others in the appropriate procedure and techniques.
