Overview
We are seeking a highly motivated, innovative, enthusiastic, and inclusive staff member to join our team as a Chronic Disease Program Coordinator.
The Bilingual Chronic Disease Program Coordinator position will be responsible for building relationships with communities across Larimer County, implementing programming to reduce the burden of chronic disease, and aligning these efforts with the CDPHE Chronic Disease grant.

Schedule
The standard work schedule for this position is Monday through Friday, 8:00 AM to 4:30 PM. During the initial six-month probationary period, the individual hired may telework up to one day per week, if business needs allow. After successfully completing the probationary period, the individual may telework up to two days per week, depending on business plans and operational needs.
Additionally, this position includes assignments that require in-community work, which may affect the hybrid remote schedule and involve hours worked outside of standard business hours. Your specific schedule will be discussed with you to ensure it aligns with the needs of the role and the business.
This position is grant-funded. The grant is renewed on a three-year cycle beginning in 2026. We anticipate continued funding beyond the current grant period; however, future renewals are not guaranteed.

The Department of Health and Environment has a vaccine requirement. Successful candidates will be required to comply with the vaccine requirements or request a waiver.
