Description:
• We are seeking a professional for a full-time position as a Children's Blended Case Manager in the Philadelphia, PA area.
• This role is onsite and requires bilingual proficiency in Spanish and English.
• The position involves extensive case management services for high-risk children and adolescents, with a focus on crisis intervention and community resource connection.
Requirements:
• Provide case management services to approximately 25 high-risk children and adolescents.
• Conduct functional assessments to identify skill deficits and strengths.
• Develop and implement service plans based on assessments.
• Connect consumers with appropriate clinical and community resources.
• Maintain case management records and provide crisis intervention services.
• Participate in interagency and team meetings.
• Adhere to electronic health record and compliance policies.
Qualifications:
• Bachelor's Degree in sociology, social work, psychology, gerontology, or related field.
• Two years of experience in human services with one year of direct client contact.
• Master's Degree preferred.
• Bilingual proficiency in Spanish and English required.
• Knowledge of crisis intervention and community resources in Philadelphia.
