Position Summary

This position offers a $5,000 New Hire Bonus!

  • Candidates must be located in Miami Dade county, in the surrounding West Miami and Doral. Applicable Zip codes 33184, 33172, or 33186.
  • This role will require 50-75% travel for face-to-face visits with members in Miami Dade.
  • Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours.

We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join

our Case Management team. This opportunity offers a competitive salary and full benefits. Our organization promotes

autonomy through a Monday Friday working schedule, paid holidays, and flexibility as you coordinate the care of your

members. Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning,

implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to

facilitate the member's overall wellness. Case Management Coordinator will effectively manage a caseload that includes

supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance

the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to member's health needs; including but not

limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication

review, Community resources and supports.

Required Qualifications

  • Bilingual, English, and Spanish.
  • Must reside in zip code 33184, 33172, and 33186
  • Willing and able to travel 50-75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL.
  • 1+ year of experience in behavioral health or a long-term care setting.
  • Computer proficiency in Microsoft Word, Excel, and Outlook required.

Preferred Qualifications

  • Strongly prefers previous Field Case management and discharge planning experience
  • Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
  • Effective communication skills, both verbal and written
  • Managed Care experience

Education

  • Bachelor's degree is required, preferably in social work or related field.

Pay Range

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
