Community Bridges is seeking a full-time Behavioral Home Direct Support Professional (DSP) in Boscawen, NH.
Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. Serving more than 1,100 individuals and families across the Greater Capital Area, we work to remove barriers, create opportunities, and foster meaningful community connections.
The Behavioral Home Direct Support Professional serves as a community connector, supporting individuals through community mapping and intentional engagement to promote social inclusion. This role focuses on building relationships, identifying strengths, and expanding opportunities that support each individual's growth, safety, and integration within their community.
DSPs promote abilities-not disabilities by helping individuals build meaningful connections with family, friends, and community members while encouraging independence and personal development.
Education
High School Diploma or GED required
Experience
Experience in human services or working with individuals with disabilities preferred

Part-Time – Wednesdays 12-Hour Shifts:
9:00 a.m.–10:00 p.m.
8:00 p.m.–9:00 a.m.
Support individuals with planned activities and community involvement
Develop relationship maps to identify social inclusion opportunities
Provide continuous supervision and intervene as needed to maintain safety
Assist individuals in building and expanding social networks
Promote self-esteem through meaningful community participation
Provide individualized supports, including personal care and skill development
Assist with hygiene supports as needed
Participate in Person-Centered Planning and goal implementation
Complete required documentation, including data collection and progress notes
Assist with orientation and training of new staff
Administer medications in accordance with He-M 1201, as applicable
Report safety, health, or human rights concerns immediately
Ensure compliance with state regulations and agency policies
Perform other duties as assigned
Ability to remain calm and respond effectively in high-risk or escalated situations

Primarily indoor residential setting
Occasional noise and high-stimulation periods

Access to a personal vehicle during scheduled work hours
