Responsibilities
Monitor and document patient behavioral health status, including mood and safety indicators.
Maintain accurate, timely documentation in electronic health records and state systems.
Monitor and manage escalation procedures for risk of harm, including safety checks.
Assist clinicians with intake, assessments, and care plan updates during shifts.
Implement behavioral health interventions under supervision, following approved protocols.
Collaborate with multidisciplinary team to coordinate care and ensure continuity.
Meet performance targets and contribute to quarterly pipelines for patient outcomes and service delivery.
Maintain clean, compliant clinical environment and adhere to confidentiality policies.
Engage in ongoing professional development and participate in training and certifications.
Support patient engagement initiatives to improve adherence and attendance.
Capture, track, and report key performance indicators related to caseload and outcomes.
Act as a role model for growth-minded behavior and contribute to a merit-based culture.
Experience/Qualifications
Ability to assess risk and execute escalation and safety procedures.
Demonstrated ability to monitor and document patient behavioral health status.
Proficiency in electronic health records and state system documentation.
Implement behavioral health interventions per approved protocols.
Track and report key performance indicators and caseload metrics.
Assist with intake, assessments, and care plan updates under supervision.
Collaborate with multidisciplinary teams to coordinate patient care.
Maintain compliant, confidential, and safe clinical environment.
