Behavioral Health Specialist (LPC or LMSW)
Are you passionate about helping individuals improve their mental health and overall well-being? We are seeking an experienced Behavioral Health Specialist to join our collaborative outpatient care team. In this role, you will work alongside primary care providers, care managers, and other healthcare professionals to support patients through counseling, treatment planning, and ongoing behavioral health services.
This is an outpatient-only opportunity with a predictable schedule, no hospital call, and the ability to build meaningful, long-term relationships with patients.
What You’ll Do
- Conduct comprehensive behavioral health and mental health assessments
- Develop individualized treatment plans tailored to each patient’s needs
- Provide outpatient counseling and therapy services to adults and/or children
- Deliver crisis intervention and behavioral stabilization support when needed
- Maintain a manageable caseload focused on quality and continuity of care
- Collaborate with physicians, care managers, and interdisciplinary teams
- Connect patients with community resources and behavioral health services
- Complete accurate documentation, treatment plans, and progress notes
- Monitor patient progress and adjust treatment approaches as appropriate
- Prepare discharge summaries and transition plans
Requirements
What We’re Looking For
- Master’s degree in Social Work, Counseling, or a related field
- Active LPC or LMSW license
- At least 3 years of outpatient behavioral health experience
- Strong background in assessments, treatment planning, therapy, and crisis intervention
- Excellent communication, organization, and teamwork skills
- Comfortable working in a structured, integrated care environment
- Dedicated to serving vulnerable populations and improving patient outcomes
