Job Summary and Qualifications

Provide a patient onboarding experience for patients admitting to the in-patient units. Provide support to in-patient units upon discharging. Responsible for assessing, planning, implementing, evaluating and documenting all aspects of patient care. He/she will work closely with all health care providers to facilitate and coordinate efficient, effective, quality health care. The individual must demonstrate knowledge of the principles of growth and development over the appropriate life span; possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age specific needs; and, have the ability to provide care needed by the pertinent patient group(s). He/she will implement and/or comply with all the policies, procedures and standards of the unit, department and the institution.

What you will do in this role:

  • Coordination of care with physicians.
  • Actively collaborates with other units and program staff, physicians and members of other departments to facilitate provision of optimal patient care.
  • Provide patient education while administering medication.
  • Practice safe medication administration consistent scanning of patient and medications.
  • Scan all medications and patients using eMar.
  • Follow facility established guidelines for reconciliation of medications on admission, during transfer and at discharge.
  • Assign and conduct appropriate individual and group interventions within the milieu for behavioral and safe management of patients.
  • Maintains appropriate boundaries.
  • Demonstrate least restrictive interventions appropriately.
  • Collaborate with interdisciplinary team in assessing, planning, implementing and evaluating treatment, patient safety and programs.
  • Assess patient’s response to treatment and safety interventions and accurately document interventions and patient’s response to intervention.
  • Assist patients and families to understand the diagnosis, prescribed treatment, and safe management of symptoms related to specific mental disorders through health teaching.
  • Document against the interventions within the Recovery Plan Utilize teaching skills related to psycho educational and medical interventions and patient safety needs with the patients and families.
  • Assumes responsibility for renewing license.
  • Assumes responsibility for completing all mandatory educational, health screening, CPR, and CPI requirements within designated time frame and for providing written verification of same for filing.
  • Transcribes orders accurately and scans orders to pharmacy in a timely fashion. Provides the new patient with an in-depth on-boarding experience.
  • Provides support to the unit staff with discharging patients. 
What qualifications you will need:
    • Basic Cardiac Life Support must be obtained within 30 days of employment start date
    • Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
    • (RN) Registered Nurse
    • Associate Degree
