Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Behavioral Health team! The Behavioral Health (BH) Program Coordinator works as part of the Behavioral Health and other teams (e.g., SUTS, patient programs, TIC) facilitating clinical integration within the primary care setting. This position drives and organizes Erie's medication-based treatment program for patients who wish to recover from substance abuse. The Program Coordinator is responsible for conducting the initial Needs Assessment, tracking patient enrollment, and serving as the point of contact to ensure adherence to, and appropriate follow up for patients within the program.

Main Duties & Responsibilities
Completes intake assessments for all new patients.
Facilitates group therapy sessions as needed.
Conducts onsite and phone follow up with patients to complete ongoing assessments.
Facilitates internal and external referrals to agencies that can assist patients with basic needs and address social drivers of health.
Works with the team supervisor to design, develop, and maintain patient tracking tools and the registry for patients. This database will have up to date information such as date/duration of last medication, dates of last UDS and medical visit, date of last substance use and EHR record number and date of birth.
Maintains patient enrollment and progress toward improved mental health outcomes.
Education:
Bachelor's degree required in a Behavioral Health discipline (e.g., Social Work, Psychology, Mental Health Counseling, etc.)
