Our Behavioral Health Consultants work closely with patients, primary care providers, and care teams to provide counseling, support, crisis intervention, and care coordination that helps patients improve their overall well-being.

In this role, you will:
Provide individual, family, and group counseling
Deliver crisis intervention and behavioral health support
Complete mental health screenings, assessments, and treatment plans
Partner with primary care providers to address mental health and psychosocial needs
Support patients with behavior change, self-management skills, and lifestyle goals
Help identify barriers to care and connect patients with appropriate resources
Lead patient education and support groups
Collaborate with care team members to coordinate patient care
Maintain timely, accurate, and confidential documentation
Participate in peer review, case consultation, and quality improvement activities
Master’s degree in Social Work with limited or full licensure, or Licensed Professional Counselor licensure required
Minimum of 2 years of Social Work experience preferred
BLS certification required or ability to obtain
Strong understanding of behavioral medicine and evidence-based treatment approaches
Comfortable working in a collaborative, primary care setting
Strong assessment, communication, documentation, and time management skills
Commitment to patient confidentiality and compassionate care
