Responsibilities/Job Description:
This position is located at our Andover and Blaine clinics.
The Behavioral Health Clinician serves as a key member of the primary care and/or specialty services team, partnering to address the overall healthcare needs of patients. This role delivers assessments, consultations, and evidence-based clinical interventions aimed at preventing, identifying, and managing psychosocial and behavioral health conditions that influence overall medical outcomes and patient well-being.
Responsibilities
- Conducts diagnostic evaluation and documents assessment in medical record.
- Conducts diagnostic evaluation and confers diagnosis as appropriate.
- Develops an individualized treatment plan of care.
- Develops an individualized plan based on age, social and cultural considerations, condition, and other relevant needs.
- Maintains required, appropriate, and timely communication and correspondence with treatment authorization and payer sources in a manner which promotes efficient and effective delivery of clinical services.
- Collaborates with patients, family, and other care team to develop a treatment plan of care.
- Provides consultation to the medical team aimed at the prevention, identification, treatment and management of behavioral health/psychosocial issues and conditions.
- Functions as an active team member as evidenced by working in a collaborative approach with primary care and specialty medical providers.
- Provide comprehensive behavioral health services to patients of all ages, ensuring age-appropriate interventions tailored to the unique needs of pediatric and adult patients.
- Understanding integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional, and psychological dimensions of health.
- Employs a primary care approach in addressing the behavioral/psychosocial aspects of routine and complex medical conditions.
- Provides brief evidence based interventions for identified mental health and substance use conditions.
- Delivery of population-based health care by providing individual, group and community-based services.
- Maintains concise, timely and accurate documentation in the patient’s medical record.
- Connects patients with community resources as needed to meet basic living and support needs.
- Effectively refers and manages collaboration and communication across the continuum of care.
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
Required Qualifications
- Master’s or doctoral level degree in counseling, social work or psychology
- 2 years experience post licensure
- One of the following
- Licensed Professional Clinical Counselor (LPCC) or
- Licensed Independent Clinical Social Worker (LICSW) or
- Licensed Marriage and Family Therapists (LMFT) or
- Licensed Psychologist (LP)
- Licensed Graduate in Social Work will be considered under certain circumstances
Preferred Qualifications
- 5 years experience post licensure
