Behavioral Health Case Manager

A-Line Staffing is now hiring a Full-Time, Hybrid, Behavioral Health Case Manager in West Central Ohio! (COUNTIES COVERED ARE CLARK, GREENE, AND MONTGOMERY). (WILL SEE 5-7 INDIVIDUALS A WEEK, REST OF THE TIME IS REMOTE).

Job Summary

This Care Manager will develop, assess, and facilitate complex care management activities for members with primarily physical and behavioral health needs. The goal is to improve health outcomes through personalized care planning, education, coordination of services, and ongoing support for members and their families.

Behavioral Health Case Manager Compensation

• The pay for this position is $39/hr + mileage reimbursement
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Behavioral Health Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position –

o Monday–Friday, 8:00 AM – 5:00 PM EST

o Training: 4 weeks, Monday–Friday 8:00 AM – 5:00 PM EST (virtual, camera-on required, no time off permitted during training)

o Must complete documentation within 24 hours of member visits

o Expected field productivity: 5–7 member visits per week

• Behavioral Health Case Manager Responsibilities

  ·         Conduct member assessments and develop individualized care plans

  ·         Coordinate care services across providers and community resources

  ·         Complete 5–7 in-person member visits weekly

  ·         Maintain timely and accurate documentation (within 24 hours)

  ·         Monitor member progress and adjust care plans as needed

  ·         Communicate and collaborate with interdisciplinary care teams

  ·         Support members and families through education and resource navigation

  ·         Ensure compliance with care management standards and timelines

  Behavioral Health Case Manager Requirements

  Education & Certification

  • Required: Bachelor’s degree in Counseling, Social Work, Nursing, or related field
  • Required: Behavioral Health experience (2–4 years minimum)
  • Required Licensure: LPC, LPCC, or LSW

  Experience

  • 2–4 years in nursing, case management, home health, or behavioral health
  • Experience managing independent field-based work preferred
