Position Title: Behavioral Health Care Planner -(26140077)

The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within Montana State Hospital.

Behavioral Healthcare Planners provide assessment, treatment planning, and coordination of services to individuals with mental health and substance abuse disorders. They work closely with patients, families, and healthcare providers to develop and implement individualized treatment plans.

Responsibilities:

• Conduct assessments and evaluations to identify patient needs and develop treatment plans • Coordinate services with healthcare providers, community resources, and other stakeholders • Develop and implement individualized treatment plans, including medication management and therapy • Monitor patient progress and adjust treatment plans as needed • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

Requirements:

• Masters degree in a related field (e.g., social work, psychology, counseling) • Experience working with individuals with mental health and substance abuse disorders • Strong assessment and treatment planning skills • Excellent communication and interpersonal skills • Ability to work in a fast-paced environment and prioritize multiple tasks
