Position Overview

Telos Health Systems is seeking a compassionate and motivated Attending Physician (MD/DO) to provide comprehensive on-site primary care services to residents in skilled nursing facilities throughout the Ocoee, Florida area.

Clinical Responsibilities

  • Provide comprehensive primary care services for SNF residents
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Perform admissions, follow-ups, and medically necessary assessments
  • Manage changes in patient condition
  • Collaborate with nursing staff, therapists, specialists, and facility leadership
  • Supervise and collaborate with NPs/PAs as appropriate
  • Participate in interdisciplinary care planning and quality improvement initiatives
  • Complete timely and accurate documentation in the EHR

Call Responsibilities

  • Participate in a structured call rotation
  • Address urgent clinical concerns via telehealth
  • Telehealth encounters may provide additional compensation opportunities
  • Supported workflows designed to maintain manageable call volume

Qualifications

  • MD or DO from an accredited medical school

  • Interest in skilled nursing and post-acute care medicine
  • Strong communication and teamwork skills
