Orthopedic At-Home Physical Therapist- Per Diem JAG Physical Therapy is seeking to hire a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic related conditions. Our mission is to help patients regain mobility, strength, function and confidence in the comfort of their own homes. This approach emphasizes not only physical recovery, but also the emotional and social benefits of re-engaging with the community. We are looking for a compassionate and motivated physical therapist to join our team and make a difference in the lives of those recovering and returning to their prior level of function. Our physical therapists will be required to evaluate and treat the patients for the duration of their plan of care in their home setting with the goal of transitioning into one of our JAG outpatient orthopedic locations for continuity of care and to improve their functional outcome measures. Bachelor’s or Master’s Degree required, Doctorate degree preferred • Licensed Physical Therapist in the state practicing in • Experience/knowledge with an EMR software is preferred • Basic computer skills • Works as a team member and individually with minimal supervision • Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care. • Additionally, completes daily billing and follows department guidelines for productivity. • Prepares for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses. • Provides education/training to patient and/or patient support system • Functional Training-This includes but is not limited to ADLs/IADLs, therapeutic activities (e.g. transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration • Provide data, note writing using Prompt EMR for all treatments provided • Attends annual JAG Billing and Coding seminar • What you’ll love about us section is based on full time employment with the company and is not guaranteed based on employment type.
