This position is tailored to specific markets or centers, requiring adaptability in responsibilities and a unique combination of clinical management, financial insight, and strategic collaboration to enhance patient care and business results.
KEY RESPONSIBILITIES
Leadership & Organizational Management:
- Analyze organizational and market priorities, trends, and objectives to formulate a clinical strategy that enhances talent and performance.
- Recruit, hire, and oversee clinical teams to cultivate a high-quality workforce characterized by strong engagement, a patient-first ethos, and talent retention.
- Maintain close communication with clinicians and care teams through weekly onsite visits, regular one-on-one meetings, and active participation in monthly leadership discussions.
- Gain a comprehensive understanding of primary care clinician performance, utilizing data effectively and providing targeted coaching to clinicians and care teams.
- Participate in regular market performance meetings focusing on clinical, financial, and operational topics such as Medical Risk Adjustment (MRA), budgeting, staffing, and clinical initiatives.
- Advocate for our senior-focused primary care strategic vision and initiatives, building relationships with stakeholders, including health plan partners, healthcare providers, and social services to enhance community care.
- Represent our organization in local communities and media engagements while collaborating with recruitment teams to establish a network of high-quality primary care clinicians.
- Utilize clinical performance dashboards and data tools for performance analysis and strategy development.
- Encourage a culture of continuous learning and professional development among clinical teams.
Clinical/Patient Management:
- Develop a strong patient-centered and value-based clinical vision that aligns care teams with excellence in patient care, teamwork, and outcomes.
- Achieve outstanding clinical performance in patient experience, quality of care, clinical outcomes, and reduction of avoidable utilization.
- Regularly review clinician charts to identify care improvement opportunities, ensuring accurate clinical assessments and precise performance improvement initiatives.
- Identify critical issues for high-risk patients during case reviews and other discussions, promoting and exemplifying clinical excellence.
- Conduct root cause analyses of care opportunities from individual, team, and systems perspectives, collaborating with clinical and operational colleagues to enhance high-reliability care.
- Ensure effective co-management of high-risk care episodes and patients with the Care Integration Team (CIT) resources for transitions of care management, high-risk patient management, and social determinants of health efforts.
- Oversee daily patient care and initiatives aimed at improving team-based key performance indicators (KPIs), such as patient experience via Net Promoter Score (NPS) and Medicare clinical quality via HEDIS, aligning with local and organizational goals.
- Provide high-quality primary care, demonstrating ownership and clinical excellence in age-friendly senior primary care, including health promotion, disease management, and complex care management.
- Dedicate 20–50% of time to direct patient care, with the remaining time allocated to administrative duties, subject to market needs and staffing levels throughout the year.
- Support clinical quality performance improvement in areas such as MRA, Stars, and prevention of unnecessary utilization through provider education and coaching.
- Encourage team-based care delivery and foster collaboration across interdisciplinary teams.
Dyad Partnership:
- Collaborate with operational leaders (Associate Operations Director and/or Market President) and Shared Services partners to align clinical and operational objectives, strategic planning, and budgeting.
- Maintain consistent communication to ensure alignment on performance, strategies, and team management, fostering unified decision-making and consistent messaging.
- Work collaboratively towards shared goals that support the mission, vision, values, and overall patient experience outcomes, managing clinic/market dynamics and engagement.
- Partner on strategic and operational insights, including budgeting, while monitoring clinical and financial performance metrics.
- Review clinician schedules and incentive plans to ensure alignment with patient care access and management objectives.
Utilize Your Skills to Make a Difference Required Qualifications:
- Graduation from an accredited medical school (MD/DO) with a valid medical license, adhering to state licensure requirements.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- A minimum of five years of experience in outpatient practice, with clinical expertise in primary care, senior health, and value-based care (VBC).
- At least three years of experience successfully leading clinicians in a leadership capacity, demonstrating impact on clinician talent, culture, and performance, along with effective collaboration with operations and support teams.
- Proficient in utilizing clinical technology platforms and resources, such as EMR systems (e.g., athenahealth, Epic, eClinicalWorks), voice documentation tools (e.g., Dragon, Abridge), and clinical evidence resources (e.g., UpToDate).
- This position is considered patient-facing and is part of our Tuberculosis (TB) screening program. Selected candidates will be required to undergo TB screening.
- Selected candidates must comply with the flu vaccine policy.

Pay Range
The compensation range provided below reflects a good faith estimate of the starting base salary for full-time (40 hours per week) employment at the time of posting. The actual pay may vary based on geographic location and individual qualifications, including skills, knowledge, experience, education, and certifications.
$310,000 - $368,500 per year
This position is eligible for a bonus incentive plan based on company and/or individual performance.
