    POSITION SUMMARY/RESPONSIBILITIES
    Responsible for the development, coordination, and management of activities within Pediatric Palliative Care, Adult Palliative Care, and Inpatient Geriatric department as assigned by the department Executive Director. These can include project evaluation plans, timelines and operational planning in support of key initiatives to provide a supportive, positive patient and family experience across the continuum of care. Champions opportunities to optimize the adult, geriatric, pediatric and perinatal patient and family experience in partnership with an interdisciplinary team. Works collaboratively within a team of Geriatric and Palliative Care clinical and interdisciplinary care providers to coordinate efforts and focus support where needed. Identifies areas in need of improvement and supports planning and evaluation efforts tailored to address the needs of patients and families with complex healthcare needs. Creates customized reporting, assists in coordination of plans, events and interfaces with various teams in support of the adult, pediatric and neonatal patient and family experience to include other Health System departments that provide clinical care and support to patients.

    EDUCATION/EXPERIENCE
    Bachelor’s degree in Nursing is required. Master’s degree in healthcare related field is preferred. Excellent organizational skills with keen attention to detail, excellent communication and interpersonal skills are required. Experience in project coordination, data analysis, reporting, and planning is required. Must demonstrate the ability to work with public contacts, establish, and maintain effective working relationships and be adaptable to high-paced clinical environment.

    LICENSURE / CERTIFICATION
    Current RN licensure in the State of Texas is required.
