Bonus for Full-Time Position: $4,000 Hiring & Retention Bonus (Limited Time Offer) 




Why This Role Matters 

We are seeking a licensed Occupational Therapist or Physical Therapist to serve as Assistant Director of Rehabilitation (ADOR). This leadership role supports the Director of Rehabilitation in overseeing the rehabilitation program, mentoring a multidisciplinary therapy team, and ensuring the delivery of high-quality, resident-centered care. This position offers an excellent opportunity for clinicians seeking growth in senior leadership. 

Reporting: This position works closely with the Director of Rehabilitation and collaborates with facility leadership, nursing, physicians, and interdisciplinary teams. 

Role Overview 

  • Assist in leading and managing the daily operations of the rehabilitation department 
  • Support and supervise Physical Therapists, Occupational Therapists, Speech-Language Pathologists, assistants, and therapy aides 
  • Support regulatory compliance and ensure accurate, timely documentation 
  • Collaborate with nursing, physicians, and interdisciplinary teams to coordinate resident care 
  • Assist in developing individualized care plans and supporting PDPM processes 
  • Monitor productivity, clinical outcomes, and departmental performance 
  • Participate in audits, care conferences, and leadership meetings 
  • Provide coverage for the Director of Rehabilitation as needed to ensure continuity of operations 




What We’re Looking For 

  • Active California license in Physical Therapy or Occupational Therapist in good standing 
  • Prior experience as an Assistant Director of Rehabilitation, Lead Therapist, or Rehabilitation Supervisor preferred 
  • Clinicians with 5+ years of experience as PT in a skilled nursing facility who are seeking leadership growth are encouraged to apply 
  • Bachelor's degree in Physical therapy or Occupational Therapy (or higher) 
  • Knowledge of PDPM, Medicare, and skilled nursing facility regulations 
  • Experience with EMR systems and regulatory compliance 
  • Strong leadership, communication, and organizational skills 
  • Ability to foster teamwork, professional growth, and a positive work environment 
  • Passion for serving the geriatric population
