As the Assistant Director of Nursing, you will work closely with the Director of Nursing to support day-to-day clinical operations, staff development, quality initiatives, and regulatory compliance. In the early stages, this role will be hands-on and patient-facing, providing direct clinical support while helping establish processes and best practices. As the business grows, the position will evolve to include broader responsibilities for clinician coaching, quality oversight, and operational support across multiple locations.

This position is based in West Palm Beach, FL and requires extensive regional travel (Indian River, Martin, Okeechobee, Palm Beach, and St. Lucie counties).

Responsibilities:

What You'll Do:

  • Collaborate with the Director of Nursing in the launch and growth of Florida home health operations.
  • Facilitate clinical oversight and ensure the delivery of high-quality patient care in accordance with agency policies and regulatory requirements.
  • Monitor compliance with federal, state, and local regulations, including Medicare Conditions of Participation, AHCA requirements, and agency standards.
  • Guide oversight of clinical documentation, OASIS accuracy, care planning, and interdisciplinary coordination.
  • Support Quality Assurance and Performance Improvement (QAPI) activities, including chart audits, infection control monitoring, incident review, supervisory visit compliance, and corrective action follow-up.
  • Participate in clinical record reviews and assist with preparation for surveys, audits, and accreditation activities.
  • Provide clinical guidance, mentorship, and support to field clinicians and office staff.
  • Facilitate onboarding, orientation, training, and ongoing competency evaluations of clinical personnel.
  • Evaluate patient acuity, visit volume, and staffing needs in collaboration with the Director of Nursing and operational leadership.
  • Conduct patient and supervisory visits as needed, including Start of Care and Resumption of Care assessments.
  • Advance the development and implementation of clinical policies, procedures, workflows, and best practices.
  • Serve as a clinical resource for staff regarding patient care, documentation, regulatory compliance, and agency processes.
  • Support the transition of licensed entities from startup phase to active patient-serving operations.
  • Collaborate with intake, scheduling, business development, and operational leaders to support growth initiatives and ensure clinical readiness.
  • Act on behalf of the Director of Nursing in their absence, as delegated and permitted by state regulations.
  • Travel throughout the market to support clinical operations, staff development, and quality initiatives.
Qualifications:

What You'll Bring:

  • Strong working knowledge of Florida home health regulations, including Florida Administrative Code 59A-8, AHCA requirements, documentation standards, supervisory visit requirements, infection control protocols, and Medicare Conditions of Participation.
  • Working knowledge of OASIS documentation, care planning, and home health quality measures.
  • Proficiency with home health EMR systems such as WellSky, HCHB, Kinnser, or Axxess.
  • Ability to mentor and support clinicians while promoting accountability and regulatory compliance.
  • Strong organizational, communication, leadership, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced, evolving environment.
  • Proficient with Microsoft Office Suite.

Education and Experience:

  • Bachelor's degree in Nursing required; Master's degree in Nursing, Healthcare Administration, or related field preferred.
  • Active and unrestricted Florida Registered Nurse (RN) license (or appropriate state that this role serves).
  • Minimum of three (3) years of clinical experience in home health, hospice, skilled nursing, or a related healthcare setting.
  • Minimum of one (1) year of supervisory or leadership experience in home health or a related clinical environment.
  • Demonstrated experience supervising skilled clinical services, including RN, LPN, PT, OT, ST, and Home Health Aide services.
  • Experience in Medicare-certified home health strongly preferred.
  • Previous experience as an Assistant Director of Nursing, Clinical Manager, Clinical Supervisor, Alternate Director of Nursing, or similar leadership role preferred.
  • Experience supporting quality improvement initiatives, audits, accreditation preparation, or regulatory surveys preferred.
  • Experience supporting multi-site or geographically dispersed home health operations preferred.

Physical Requirements:

  • Ability to move, transport, or position up to 50 pounds
  • Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
  • Ability to communicate effectively and clearly with others to exchange information.

Travel Requirements:

  • Regular travel on a daily or weekly basis required, even in inclement weather
Pay Range: USD $80,000.00 - USD $95,000.00 /Yr.
