Job Summary/Objective:
The ACT Specialist collaborates with the ACT Team to provide essential services and support to maintain a client's ability to function in public settings. The recovery and person-centered practice ideals are the foundation of ACT services, which are tailored to meet the client's needs.
Essential Duties and Functions:
- Adheres to the ACT evidenced base model.
- Recognizes the ACT model outcome measurements.
- Attends and actively participates in daily report meetings.
- Maintains productivity requirements / timeliness of documentation.
- Actively connects customers to appropriate services needed.
- Participates in ACT on-call rotation and provide direct crisis intervention services.
Qualifications:
- Master’s Degree in Psychology, Social Work, or related field with Licensure in the State of Michigan as a Social Worker (LMSW/LLMSW); Limited Licensed Psychologist (LLP/TLLP); Professional Counselor (LPC/LLPC) preferred. Bachelor’s Degree with Licensure in the State of Michigan as a Social Worker (LBSW) mandatory.
- Must obtain training on the Basics ACT Programs and Principles, through ACT specific training resources within ninety (90) days of hire; and shall participate in annual training specific to the ACT model.
- Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
- No criminal history that would preclude employment under federal and state laws and regulations in a federal or state health program.
- Must support a trauma informed culture of safety to aid customers in their recovery process.
- Two (2) years of experience in psychology, social work or related field working with individuals experiencing chronic and persistent mental illness preferred.
- Specialized training, skills, and abilities: Lived experiences with behavioral issues are desired.
- One year of computer experience – Word, Excel and electronic medical record software preferred.
