Job Duties:

  • The APS Case Manager performs professional case management services for vulnerable adults in accordance with DSS policy and the APS ComPASS Model. This position assesses risk, provides protective interventions, and coordinates services to ensure adult safety while respecting the individual's right to self-determination. The role involves managing a caseload, collaborating with community partners and legal systems, and supporting emergency response efforts to maintain continuity of care. Will assist counties not primarily assigned to as needed and requested.
  • Performs case management for an assigned caseload of vulnerable adults, including adult contact, home visits, service planning, and coordination of legal proceedings as appropriate, in accordance with DSS policy, APS ComPASS Model, and established procedures. Conducts timely assessments to determine vulnerability, abuse, neglect (including self-neglect), and exploitation, and provides protective services while respecting the adult's right to self-determination and coordinating with the adult to achieve their wishes. Treat every adult with dignity and respect. Maintains accurate, objective, and timely documentation in electronic case records in accordance with APS guidelines. Assists other counties as needed and required to be on call during nonbusiness hours.
  • Recommends and coordinates, with the vulnerable adults input, necessary services in partnership with the adult to ensure safety and stability, including temporary and long-term placements and community-based supports.
  • Collaborates with law enforcement and/or court systems to support emergency protective actions when necessary. Assists vulnerable adult, after hearing their desires, with securing appropriate housing, placement, or protective arrangements to ensure client safety.
  • Coordinates communication with community partners, individuals, families, and internal staff to support service delivery and maintain effective working relationships.
  • Assesses with medical and legal support the adult's ability to secure long-term services and supports, including facilitating access to financial resources, guardianship or conservatorship, emergency services, and coordination with probate or family court.
  • Provides support during emergency shelter operations, including Red Cross shelter duty or other assigned disaster response services. Record, monitor and assist with development of adults emergency plans.
  • Performs other duties as assigned.

Minimum and Additional Requirements

A Bachelors Degree in Social Work, Behavioral Science, or Social Science.

A Bachelors Degree in any other field and one (1) year experience in a related field. A Masters Degree may be preferred for select positions.
