Facility Bemidji Main Clinic Location Bemidji MN Address 1233 34th St NW Bemidji MN 56601 USA Shift 8 Hours - Day Shifts Job Schedule Full time Weekly Hours 40.00 Job Summary The Internal Medicine Nurse Practitioner NP provides advanced nursing care to adult patients with acute chronic and complex medical conditions. Working in collaboration with physicians and healthcare teams the NP performs assessments diagnoses develops and implements treatment plans and manages patient care to promote optimal health outcomes. Conduct thorough patient histories and physical examinations. Diagnose and manage acute and chronic illnesses such as hypertension diabetes COPD heart disease and other internal medicine conditions. Order interpret and act on diagnostic tests including labs imaging and screenings. Develop and implement individualized treatment plans including prescribing medications and therapies in accordance with state laws and organizational policies. Provide patient education on disease prevention health promotion and management of chronic conditions. Monitor and evaluate patient progress and response to treatment adjusting plans as necessary. Collaborate with physicians specialists and other healthcare providers to coordinate care. Document all patient encounters treatments and progress notes accurately in the electronic health record EHR. Participate in quality improvement initiatives and stay current with evidence-based practices in internal medicine. Maintain compliance with regulatory and organizational standards for patient care. Qualifications Masters or Doctorate degree from an accredited Nurse Practitioner program. If APRN was licensed prior to August 1 1995 masters degree in nursing is preferred. Experience in internal medicine or related adult primary care setting preferred Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice. Current Drug Enforcement Administration DEA permit to prescribe controlled substances in each state of practice. State Controlled Substance License if required by State National board certification as a Family FNP-BC or FNP-C Adult-Gerontology Primary Care AGPCNP-BC or A-GNP Adult ANP-C Additional certifications require approval from a market executive leader Adult-Gerontology Acute Care AGACNP-BC or ACNPC-AG Acute Care Adult ACNP-BC Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.
